Service Level Agreement (SLA)

Corporate Customers Only

Our Service Level Agreements (SLAs) are tailored, role-specific health surveillance plans designed to meet the unique needs of your workforce. A well-structured SLA ensures clarity between Occupational Health (OH) providers and employers, outlining service requirements, performance standards, and compliance measures. Our expert team ensures industry-standard compliance, delivering occupational health services that align with your business objectives.

Further Information

An SLA is essential for all occupational health providers and employers to define the scope and standards of services provided. Key elements of an SLA include:

  • Confidentiality: Ensuring the protection of workers’ medical information
  • Record Management: Maintaining accurate and secure medical records
  • Information Sharing: Defining what health information is shared with the employer

Pathways Occupational Health will develop an SLA that includes:

  • Commencement Date and Review Date: Clear start and review periods
  • Health Surveillance and Wellbeing Assessments: Tailored to specific workplace risks
  • Risk Assessment: Evaluation of exposure to noise, chemicals, carcinogens, irritants, and vibrating tools (HAVS)
  • Operational Hazards: Assessment of machinery, manual handling, vehicles, and tools
  • Workstation Assessments: Review of desk heights, seating, ergonomic factors, screen time, and disability needs for office-based employees
  • Medical Histories: Consideration of previous health surveillance, pre-employment medicals, and additional certifications

  • Pathways Occupational Health will request data sheets confirming workplace hazards
  • Upon receiving the information, the SLA is created and sent to the company within 5 working days

The Clinical Lead will complete the SLA

  • A formal agreement is established, outlining the services Pathways Occupational Health will provide
  • A review is required every 2 years or sooner if there are changes in workplace processes or policies

  • Complete the questionnaire and any additional paperwork provided by Pathways Occupational Health
  • Provide detailed information about workplace hazards and employee roles
  • If preferred, request a face-to-face site visit to discuss the SLA
  • If a site visit is required, specify the necessary PPE for Pathways Occupational Health staff

  • SLAs play a critical role in occupational health, aligning with industry standards such as SEQOHS (Safe Effective Quality Occupational Health Service)
  • Completing the SLA establishes clear communication channels, defines roles and responsibilities, and fosters strong relationships between OH providers and employers
  • Employers should respond promptly to any issues and schedule regular contract review meetings to monitor performance, identify trends, and address concerns
  • Failure to provide accurate information may prevent the occupational health professional from fully assessing workplace risks, delaying necessary interventions
  • If you would like a face-to-face SLA completion onsite, please contact a member of our team to arrange the visit
