If you’re planning to work at sea, securing your ENG1 medical certificate is a crucial first step. Whether you’re a recruit or an experienced mariner, this mandatory health assessment confirms you’re fit to work in a maritime environment, protecting both you and your crew.

At Pathways Occupational Health, we offer a full range of medical assessments, including ENG1 equivalents and offshore fitness medicals, carried out by qualified occupational health professionals.

In this guide, we break down everything seafarers need to know about the ENG1 medical.

What Is the ENG1 Medical?

The ENG1 medical certificate is the official health assessment for UK seafarers, issued by doctors approved by the Maritime and Coastguard Agency (MCA). It confirms whether an individual is medically fit to work at sea and is required for:

  • Anyone working on a UK-flagged commercial vessel
  • Offshore workers travelling via a vessel to installations
  • Those undertaking training or employment on boats, ships, ferries, or yachts

If you’re working on water, it’s very likely you’ll need a valid ENG1 or equivalent.

What Does the ENG1 Medical Involve?

The ENG1 assessment is designed to evaluate your physical and mental fitness for working in offshore or maritime environments. It typically includes:

  • Medical history review
  • Height, weight, and BMI check
  • Blood pressure measurement
  • Vision and hearing tests
  • Urinalysis
  • Lung and heart function checks
  • Mobility and musculoskeletal assessment

The aim is to detect any underlying health conditions that could impact your ability to work safely offshore or respond in an emergency at sea.

How Long Does an ENG1 Certificate Last?

If you’re found fit for duty with no restrictions, your ENG1 certificate will usually be valid for two years. In some cases, where a condition needs monitoring or further review, the certificate may be issued for a shorter period.

You’ll receive a paper certificate immediately following the assessment, which must be kept on board when working.

What If I Don’t Pass?

If your medical identifies a condition that affects your fitness for sea duty, you may:

  • Receive a temporary certificate with restrictions
  • Be referred for additional investigations
  • Be deemed unfit for offshore or maritime work

However, in many cases, health concerns can be addressed through treatment or reasonable adjustments, allowing for reassessment at a later date.

ENG1 vs Other Offshore Medicals

While ENG1 is specific to maritime roles regulated by the MCA, other offshore workers (such as those in the oil and gas sector) may require different fitness assessments, including the OEUK (formerly OGUK) medical.

At Pathways Occupational Health, we provide a wide range of regulatory and offshore medicals, including:

  • ENG1 equivalents
  • OEUK/OGUK medicals
  • COSHH, asbestos, and lead medicals
  • Confined space and working-at-height assessments

We’ll work with you or your employer to ensure you receive the correct assessment for your role.
